In 2021, a study conducted by Hickman and colleagues on 10,000 young people aged 16 to 25 in ten countries, published in *The Lancet Planetary Health*<\/a>, found that 75% considered the future \u201cfrightening\u201d and that the emotional response to climate change was \u201crational and does not imply mental illness.\u201d<\/strong>The World Health Organization\u2019s ICD-11<\/a>, which took effect in 2022, does not include eco-anxiety as a standalone diagnosis. In public discourse, the term is often used differently: to describe a psychological condition requiring treatment, rather than a rational response that should be heeded. <\/p>\n\n

The term \u201ceco-anxiety\u201d first appeared in an institutional document in the American Psychological Association\u2019s (APA) 2017 report<\/a> on the mental health risks associated with climate change. The operational definition in the field of mental health is: chronic concern about the fate of the environment and the consequences of climate change. The scientific literature distinguishes between two forms. In the first, eco-anxiety is an adaptive response: an emotional signal proportionate to a real and documented threat, which can motivate action. In the second, it becomes a clinical disorder: persistent and paralyzing states of anxiety that require therapeutic support. Both patterns are real. They are not the same thing. <\/p>\n\n

The distinction becomes problematic in the second context of use. When a rational emotional response to a real risk is labeled \u201canxiety,\u201d the frame shifts: it moves from the political arena to the clinical one. In this frame, the person concerned about the climate is not responding to scientific data: they are suffering from a disorder. The World Health Organization addresses climate-related mental health in the context of the response to the climate emergency: WHO reports recognize psychological consequences as a direct effect of global warming, alongside heat-related deaths, infectious diseases, and economic losses. The institutional approach is not to medicalize concern about the climate, but to address its causes. In Italy, the National Climate Change Adaptation Plan (PNACC 2023)<\/a> recognizes the impacts of climate change on human health as one of the risk areas to be addressed with specific measures, without treating eco-anxiety as a clinical diagnosis separate from the conditions that cause it. <\/p>\n\n

In the Italian public debate, \u201ceco-anxiety\u201d appears in two different contexts. In the first, it accurately describes the condition of those who need therapeutic support to manage a response that has become clinically significant. In the second, it is used to describe the entire emotional response of young people to climate change\u2014including rational and proportionate reactions\u2014as if the problem were the feeling itself rather than the situation that causes it. In this usage, the focus shifts from the legitimacy of the concerns to the psychological stability of those expressing them. The content of the requests is not discussed; instead, the condition of those making them is described. <\/p>\n\n

It must be acknowledged that the distinction between an adaptive response and a clinical disorder is not always clear-cut. There are documented cases in which eco-anxiety manifests as clinically significant conditions: panic attacks, insomnia, and difficulties with daily functioning<\/strong>. In these cases, psychological support is appropriate and necessary. The documented problem is not that psychological support is wrong; rather, it is that the term is used indiscriminately, applied to both adaptive responses and clinical disorders, making it difficult to distinguish between those who need therapy and those who need their concerns to be taken seriously as a warning sign. <\/p>\n\n

This column explores climate-related terms whose technical meaning differs from their common perception. \u201cEco-anxiety\u201d introduces a new mechanism: not a technical term that loses precision when translated into everyday language, but a term with two distinct and legitimate uses (adaptive response \/ clinical disorder) that, in public discourse, is applied indiscriminately, shifting the rational response to risk from the realm of political action to that of individual care. <\/p>\n\n

The next time you read \u201ceco-anxiety\u201d in an article or press release, there\u2019s one question to ask: Does the term describe a clinical disorder that requires therapeutic support, or does it describe a rational response to climate change that calls for action on its causes?
